REQUIRED:

I was a member of the East Valley Pretty n Dink team, which went undefeated throughout the season and won the State Championship in the 3.5 50+ division. During the championship, I remained undefeated and participated in over 70% of my team’s matches.

Given this performance, I believe it is reasonable to be considered for a 4.0 index rating for APPL. I would also like to continue playing with my team as we move up to the next division.
